Mersey Care NHS Foundation Trust has a number of Physiotherapists who cover a number of departments/services. In the first instance the post holder will be working within The Liverpool Out of Hospital Service.

The Liverpool Out of Hospital Service is currently growing its Intermediate Care urgent response services composed of Therapies, including Occupational Therapy, Physiotherapy, Therapy Assistants, Social Work and Nurses, supported by Health Care Assistants. The whole service has Band 4 and Band 3 Assistant Practitioner support who have therapy and nursing competencies. The disciplines are integrated to deliver timely assessments, care and rehabilitation to adults in a variety of settings city wide.

Our staff work to ensure that the needs of patients are addressed in a safe and timely manner in the patients own home to prevent unnecessary hospital admission, or responding to patients who require timely support on their discharge from hospital to maximise their independence, by provision of therapeutic interventions at home, or on their pathway home via our Intermediate Care beds.

In addition to the model of care above the Liverpool Intermediate Care Service has been commissioned, in partnership with the Royal Liverpool University Hospital and other partners, to deliver the Integrated Frailty Service – a supportive discharge service to patients identified as Frail using the Bournemouth Criteria. The patients pathway starts at Accident and Emergency, with effective Triage identifying anticipated length of stay of up to 72hours and the patient is stepped up to a Consultant led Frailty Unit. You will work as part of a proactive multi-disciplinary team (MDT) in the acute hospital trust, intermediate care beds and community setting and communication is crucial to deliver patient-centred care for this service

To ensure the safe transition from the acute to community setting patients are discharged home in an ambulance and a Therapist will review the patient on the same day, or the patient is discharged home in a taxi with a Therapist. Patients are then assessed at home using the Discharge to Assess (D2A) model. Therapy input in the community is delivered for up to 5 days. This allows us to establish goals and optimise the patient physically and functionally to ensure the patients safety at home, promote independence and prevent a further hospital admission.

The post is for 37.5 hours per week. The post holder will work between the core hours of 8am and 8pm on a rota system which involves weekend working.

Post holders will require relevant experience of working as part of an MDT and also as a lone practitioner in the community. They should be able to practice autonomously, have skills in risk assessment and management and demonstrate sound evidence based clinical decision making at the appropriate level.

The post holder will be required to support the management of a defined caseload whilst also being responsive to new referrals for patients in crisis. Skills in triage, time management, prioritisation and delegation are essential.

The post addresses a wide variety of medical conditions seen in several different environments with a requirement to be flexible and to maintain timely response to patient needs. It is therefore necessary to have the ability to travel across the city independently for work purposes.

Here in the Liverpool Out of Hospital Service and across the Mersey Care NHS Foundation Trust we work hard to ensure our staff are valued and well supported. We deliver monthly In-service training as routine within the MDT which gives opportunity for all bandings of staff to share good practice and to develop invaluable presentational skills.
