Bristol – Various Locations
Salary aligned to NHS Agenda for Change

Bristol Community Health employs over 1,800 staff, the majority of whom are highly skilled clinicians, working in a wide variety of settings. We’re the largest provider of Community Healthcare in Bristol, with over 40 NHS funded services – from physio to podiatry, diabetes to dermatology, and nursing to therapy. Our Physiotherapists work in a range of teams and non-hospital settings. Our Integrated Therapies Teams include mobile teams providing rehabilitation in patients’ homes and a Community Beds team who support in-patients in Intermediate Care Centres and Care Homes. Whichever team you work in, you will be an integral part of care in our community.

Within this role, you will join one of our Community Therapy Teams covering one of three localities in Bristol, providing assessment and rehabilitation in peoples’ own homes as part of an experienced and skilled multi-disciplinary team. Working alongside Occupational Therapists, Therapy Assistants and Community Nurses, you will ensure the safe rehabilitation of recently discharged patients, supporting them to reach their maximum level of independence.

With the support of the multi-disciplinary team, you will be confident in providing rehabilitation to patients following a range of acute illnesses including stroke/brain injury, progressive neurological conditions, amputations, orthopaedic trauma and infections.

You’re a good communicator, compassionate, collaborative, resilient and are committed to providing patient centred care.
